PROGRAM SUPERVISOR
HOUSING PROGRAM & SUPPORTED EMPLOYMENT PROGRAM

THE ROLE:
POSITION SUMMARY: The Program Supervisor oversees the Residential Housing Program and the Supported Employment Program as well as some aspects of the Agency Resource Room. Position works to provide housing and employment services to adults with severe mental illness. Position coordinates with the grants, contracts and the Oregon Supported Employment Center for Excellence to adhere to evidence-based fidelity. Responsibilities include helping individuals with a mental illness seek, gain and maintain employment and housing as well as working to develop opportunities for clients in the community. Position coordinates with the State’s Vocational Rehabilitation Counselor for eligibility determination and case planning. Position also is responsible for maintaining appropriate records; carrying out relevant Oregon Supported Employment Center for Excellence, Vocational Rehabilitation, and LCS agency policies.

ESSENTIAL JOB FUNCTIONS/MAJOR JOB RESPONSIBILITIES:
Oversees the program areas of housing and supported employment. Directs staff to maintain lists of available low income housing and establishes strong relationships with apartment managers and landlords. Evaluates housing needs based on data (family composition, number of bedrooms needed, etc.). Completes and submits applications prior to arrival. Acts as liaison to apartment managers and landlords and represents LCSNW and the programs and services in a professional manner. Secures housing in accordance with contract terms and guidelines. Closely coordinates housing activities with case management staff and program director. Reviews leasing agreements post-arrival, if necessary and assists with post-arrival/move-in problems, if necessary Model and encourage direct, professional communication among staff members and facilitates development of a positive staff support network. Demonstrates awareness and sensitivity in the delivery of client services as it relates to the cultural and socioeconomic characteristics of the populations served. Works with Vocational Rehabilitation counselor on referrals of individuals to program. Works with individuals eligible for services on vocational exploration including: assessment of skills, abilities and interests, work plan development.
Works with individuals on job readiness assistance such as: identification of barriers to employment, discussion of appropriate work behaviors, classroom activity coaching,preparations for: food handler’s certification, GED certification, Driver’s license certification as appropriate, etc. Assists with life skills which can help an individual be successful in the workplace. Assists client on completion of applications such as Social Security, job applications, housing, application for Vocational Rehabilitation, etc Assists clients with resumes and cover letters for job applications Provides information about and referral to community employment and vocational resources.
Provides direct support to assist individuals achieve work goals
Works with possible employers on job development tailored to the needs of the individual. The expectation is that individual will make at least 5 face to face employer contacts per week that are client specific. Works with employer and individual to help individual achieve success in the work place. Works with individuals in community settings as needed and appropriate. It is expected that at least 50% of scheduled work hours will be spent in the community engaged in promotional activities with prospective employers, community partners, speaking engagements etc. Provides case coordination/planning between Vocational Rehabilitation and LCS Completes necessary paperwork including documentation of client progress toward plan goals Completes monthly service report for OVRS and quarterly outcome report for Oregon Supported Employment Center for Excellence. Acts as an advocate for individuals in community as needed to assist with program success Models and encourage direct, professional communication and facilitate development of a positive community network. Demonstrates awareness and sensitivity in the delivery of consumer services as it relates to the cultural and socioeconomic characteristics of the populations served Other duties as assigned SUPERVISORY RESPONSIBILITY: Supervises program staff and conducts performance evaluations.

SECONDARY FUNCTIONS:
Provide monthly report to program director Participate in case management staff meeting Maintain on-going contact with families Attend all-staff meetings, other meetings, and trainings as requested by supervisor Other duties as assigned by supervisor EDUCATION and/or EXPERIENCE: A Bachelor’s degree and QMHA certification is required.

CERTIFICATES, LICENSES, REGISTRATIONS: A valid driver’s license, car insurance, and access to a vehicle are required for this position. QMHA
