Community Based Treatment Provider Level II or III

Job description          

This position can be filled as either a level II or III and can be located in Warsaw or Batavia.
The Community Based Treatment Provider III (CBTP) performs functions that assist in the care and treatment of children/youth and families and assisting children/youth to develop life and social skills in a community based setting and in establishing a safe physical and emotional environment for children/youth and families in care. The CBTP delivers direct services in CFTSS programs that assist the children/families in meeting their goals/desired outcomes. Services include CPST, PSR, Skill Building, Planned Respite, Family Caregiver and Support Services, Community Advocacy Services, Vocational Services/Supported Employment, Special Needs Community Advocacy and Support, Crisis Intervention and Management Services, Psychosocial Rehabilitation, and Habilitation. The CBTP may need to complete initial assessments on Risk, health and create a Treatment Plan that is reviewed every 90 days.

Level II requirements: High school/GED education and at least 2 years experience.
Level III requirements: Master’s degree in a Human Service related area (Social Work, Mental Health Counseling, etc.) and 2 years’ experience working with children who have behavioral, developmental disabilities or mental health issues. required. Will consider candidates with a Bachelor’s degree and 10 years’ experience.

Candidates must be able to travel to Wyoming, Sothern Erie and possibly other counties in Western NY based on need.
