Founded in 1887, the San Diego Center for Children is the oldest children's 501(c)(3) nonprofit in San Diego. The Center provides evidence-based therapeutic, educational, foster care and transition services to children and families struggling with mental, emotional and behavioral disorders. Today, with 8 program sites and community-based services within hundreds of homes and schools across San Diego County, the Center empowers over 1,000 people every day.
SUMMARY Youth Partners will provide therapeutic services and support to youth and transitional aged youth (TAY) in wraparound teams and individual rehabilitation meetings; Collaborative partnerships with all wraparound team members and community partners are critical; Average caseload 10 - 12 youth. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. (Other duties may be assigned) Provides crisis intervention, case management, rehabilitation and other therapeutic services to youth on your caseload, with the goal of supporting youth self-advocacy, social connectivity, and placement stability Acts as a mentor and positive role model, and assist in engaging youth in the wraparound and therapy processes Provides culturally competent education and hands-on coaching to youth to help them better understand how the “systems” work, and the process of accessing community resources Aligns with and helps support youth in to navigating through the system of care Assists teens in completing Wellness Notebooks Participates as the youth’s advocate at meetings Locate, coordinate and develop linguistically and culturally diverse informal and formal resources and networks for youth in the communities where they reside Promote greater youth participation in policy, program and service delivery in the WrapWorks Program and in the mental health system Serve on Program Advisory Group (PAG), agency committees, and task forces, as assigned Utilizes supervision, consultation, and training to provide high quality services to youth in an effective manner Maintains accurate, timely, and complete case records per Medi-Cal requirements and SDCC policies and procedures Maintains clear, effective, and open communication between multi-disciplinary clinical staff, and administrative personnel within and between SDCC programs Availability to work evening and occasional weekend hours, and to be on-call as needed by youth on their case load during working hours Fulfills all required training including CMHS Family Partnership Training, applicable EBP training, cultural competency, and HIPAA LANGUAGE SKILLS Ability to read, analyze, and interpret medical records, journals, reports, and legal documents; Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community; Ability to effectively present information to management and public groups REASONING ABILITY Ability to read, analyze, and interpret medical records, journals, reports, and legal documents; Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community; Ability to effectively present information to management and public groups PHYSICAL DEMANDS The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. WORK ENVIROMENT The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of the job. While performing the duties of this job, the employee is occasionally exposed to outside weather conditions. The noise level in the work environment is usually moderate.
MINIMUM REQUIREMENTS: Valid California Driver’s License High School diploma or GED Experience as a consumer of Children’s Behavioral Health Services, Child Welfare Services, the juvenile justice system, or special education Good verbal and written communication skills, and the ability to work effectively within a multi-disciplinary team Must have a good driving record and reliable transportation Must be 21 years of age or older PREFERRED REQUIREMENTS: Associates Degree or higher education from an accredited university Proficient in Anasazi (online medical records system) Experience as a youth advocate, educator, or mental health counselor Bilingual in Spanish
